Credit Card Authorization
Arrangements can be made on our website or completing the required forms and authorization form out of town, this easy form will enable you to make payments via fax or email. Affordable Cremation & Funeral Services of Ohio accepts all major credit cards.

The Affordable Cremation & Funeral Services of Ohio staff will help you submit the tribute to the relevant newspapers, internet social media sites and share it from the Online Obituary section of our site.
These documents are fillable PDF forms, meaning, they may be filled-in using your computer. Click with your cursor above any available line on a form and you will be able to type in the requested information. You may also click on any of the checkboxes to mark them. Once all fields are completed, email or print out the document on your printer and physically sign the document where indicated, if necessary. The Next of Kin form must be printed, signed and notarized.
